File A Claim

Pursuant to the Court's Order Granting Preliminary Approval Pursuant to Fed. R. Civ. P. 23(e)(1) and Permitting Notice to the Class, dated March 16, 2023, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release in accordance with the instructions contained therein. You may submit your Proof of Claim and Release using any one of the following methods. Each method is described below, with links to the requirements for each submission. Your submitted Proof of Claim and Release will include a certification under penalty of perjury that your Proof of Claim and Release is accurate and complete.

Whichever submission method you choose, you are required to provide documentation to support your claimed transactions. Please remember to provide supporting documentation of all transactions listed on your Proof of Claim and Release. In the event that you submit your Proof of Claim and Release online, you must retain confirmation of such filing.

If you fail to execute and submit a timely, properly completed and addressed Proof of Claim and Release, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.

To submit a Proof of Claim and Release by mail, complete and sign the Proof of Claim and Release in its entirety, with all required supporting documentation and mail it, postmarked no later than July 7, 2023, to the Claims Administrator at:
